Master Series 2013:  Bringing Healthcare Home

 

Issues related to Post Acute Care, Long-term, Palliative, SNF and Hospice Care. Planning and design of innovative healthcare solutions geared toward end of life and chronic post acute and rehab care.  This session will explore of the evolving role of rehabilitation and post-acute care within the continuum of care.  Discussion will include strategic and operation issues that affect planning and design for long-term acute care, rehabilitation, skilled nursing facilities, and home health facilities.

Next Practices: A Deeper Dive

The mission of the ACHA Next Practices Committee is to identify forward thinking practices that will contribute to making healthcare more affordable, accessible, and improve the quality of patient care. At HCD12, we presented multiple concepts including the need for the integration of public health policy and urban design, the “devolution” of care, genomics/precision medicine, nano medicine, tissue engineering, mobile electronics and lab-on-a-chip. This year we will closely examine three of the concepts, including what’s the “so what,” timelines, how they will affect the delivery of care and impacts to facility planning.
